What We Do

The Long-Term Home Health Care Program offers families and individuals the opportunity to remain in their own homes and receive a range of health care and supportive services.

Each participant receives a full assessment of his or her needs, and an individualized plan is developed and monitored to assure that care is coordinated and complete.

Essentially a “nursing home without walls,” the Long-Term Home Health Care Program offers an alternative to institutional placement for seniors and chronically ill individuals.
